Annual Memberships
I provide both your medical care and also service "enhancements" which are not covered by health care insurance. Current federal laws, state laws and insurance company regulations create a situation in which I must work for you under two different names.
"The doctor" providing your "covered" (by insurance) medical services is William C. Daly M.D., LLC, and I bill your insurance company directly.
The advantage of this practice is that I also provide you service enhancements NOT covered by your health care insurance. This includes long, relaxed appointments giving you ample time to address your concerns, and seeing you on time with usually no waiting. I am available to you 24/7 with direct access by cell phone, e-mail and landline. Appointment times are flexible, with urgent visits the same day you call. I also provide electronic prescribing, nutrition counseling and other advantages, all under the name "Andover Medical Associates”.
To make these personalized services possible and still be available for each you, I must limit the number of patients. Andover Medical Associates, LLC, charges an annual membership fee for these specified "amenities" that are not covered by your health insurer.
Annual Membership Fee:
• Individual Age 16-24………... $900 / year
• Individual Age 25-34………. $1200 / year
• Individual Age 35-64………. $1800 / year
• Individual Age 65+.………... $2400 / year
• Adult Couple Age 35-64….. $3300 / year
• Senior Couple Age 65+.…… $4500 / year
• 1 Senior + 1 Age 35-64…… $3900 / year
Patients have overwhelmingly found this to be an excellent value – the improved quality of their healthcare far outweighs the cost. If you calculate this fee on a monthly basis, it is similar to what many people pay for their cable TV/internet connection, restaurants, or movies. And it is less than the cost of a few beers, one mixed drink or smoking one pack per day – while producing far more benefit for your health and well-being!
Payments may be made by check or credit card, on an annual, semi-annual or quarterly basis, understanding that the Membership Agreement is an annual contract. Discount $100 for membership paid in one annual payment.Also discount $300 from the age-related individual rate for the third and additional family members. Please make payment to "Andover Medical Associates." Andover Medical Associates, LLC is not an insurance company or product.
You do need to maintain your healthcare insurance to cover tests, specialists, hospitalizations and other covered services. My office visits and hospital care charges are billed to your health insurer as usual, by my professional practice entity William C. Daly, M.D., LLC. I am very sorry that current laws and insurance regulations require that you remain responsible for copayments and deductibles. Governmental laws and insurance company policies mandate that the membership fee is only permitted to pay for services and amenities not covered by your health plan.
